To be eligible for funded primary healthcare in New Zealand you must meet certain criteria.  We can advise your eligibility if you are unsure.

Please review the information on Enrolling with General Practice and PHO, along with our Health Information Privacy Statement. prior to completing your enrolment to ensure you understand what you are agreeing to.

Please fill in an enrolment form and provide evidence of eligibility.  Please email the form, along with a copy of your NZ Passport or Birth Certificate (or other passport and appropriate visas) or bring them in to the clinic. Non-NZers will be required to provide a copy of their passport and appropriate visas prior to confirmation of eligibility.  Drivers Licences are not an acceptable form for eligibility.  

Patients over the age of 16 are able to book appointments, request repeat prescriptions and view their lab results online. Please note that each person requesting online access needs a unique email address.
